Terms and conditions for Fit4Baby Membership

FIT4MOM PLACER FIT4BABY MONTHLY MEMBERSHIP AGREEMENT

This contract is made between the client and FIT4MOM PLACER and is a month to month agreement and except as provided in the initial 10 day cancellation policy below, may be terminated by either party with the proper advance notice. This membership allows you to attend Fit4Baby classes within the FIT4MOM PLACER franchise.

AUTHORIZATION FOR PAYMENT
I authorize FIT4MOM PLACER to initiate a charge to the account entered into Front Desk for my dues. This authorization is to remain effective until I have cancelled my membership through the FIT4MOM PLACER administrator. Charges will be billed from SMITH FITNESS.

I have the right to stop payment on an Electronic Funds Transfer debit by notifying my bank. This, however, does not void my contract with FIT4MOM PLACER to fulfill my payment commitment and I am obligated to pay. Dues will be drawn monthly from the account on file on the 1st day of the month or when the membership is initially started. The processing date for debit cards may vary due to banking procedures. If charges are returned they will be subject to a late fee of $25. Members are responsible for updating their credit card information online. Members may not attend class who have unpaid balances. If a membership remains unpaid it will be cancelled.

CANCELLATION POLICY
To cancel this membership, the member must contact the FIT4MOM PLACER administrator 30 days prior to their next payment due date to request a cancellation. If less than 30 days notice is given, a cancellation fee of $25 will be added to the ending balance. No refunds will be given for dues once it has been paid. This membership is not transferable to another person or anywhere outside the FIT4MOM PLACER franchise. Once a membership is cancelled, if the member wants to re-enroll they will be charged half the normal registration fee if reactivation is within one calendar year. If the member wants to re-enroll but membership has lapsed for more that one year, the full registration fee will be charged upon re-activation.

HOLD POLICY
To place a hold on this membership, the member must contact the FIT4MOM Placer administrator 30 days prior to their next payment due date to request a hold. If less than 30 days notice is given, a hold fee of $25 will be added to the account. Memberships may be held a maximum of 2 times per calendar year and for up to 4 months total. Holds can start and end on any day of the month. When a hold period is ended, the monthly auto payments will resume on the next eligible class date. There is no fee for holds if 30 days notice is given before the hold begins.

INITIAL 10-DAY CANCELLATION POLICY
You, the buyer, may cancel this agreement at any time prior to midnight of the tenth (10th) calendar day after you have signed this agreement. To cancel this agreement written notice must be received by FIT4MOM PLACER within the initial ten days. You are responsible for paying for the classes already attended.

AGREEMENT
By purchasing this contract, you are agreeing to the terms of this contract. Terms of this agreement may change with proper advance written notice.
